It could be a sticker, or it could be on a bit of string. Either is possible.
Incidentally, I think the tone of command used in this sentence is too harsh. It should probably be 荷札を付けて or something.

Actually, either the English or the Japanese could have been the original. There's no way to tell for certain, although I suspect the English came first in this case.
I remember somebody sneered at me when I said I could detect a japanese original...but I can't remember who that was...
Doesn't the rest of the sentence sound too stiff for that, though?
