Müsste es nicht eher heißen: "Er sieht überhaupt nicht wie ein Arzt aus." ? Ist nicht im Englischen "a doctor" eher ein Arzt als ein allgemeiner Inhaber eines Doktortitels?
Does "a doctor" mean "someone with a Ph.D." or "a physician/medic"? (If medic, then in German it should probably be "ein Arzt".)
The Japanese is 'physician/medic', and the English would probably be interpreted in the same way.
