まあ、熱のない場合にはふつう「熱が出たら」って言いますね。

熱を計って熱があるのをもう知っている、という場合にしておいて訳しましょうっと。

I ended up translating it as if both the speaker and the patient already know about the fever.

確かに熱がないと知っているなら使えませんね。

まだ知らないならありえます。
「熱は測ったの? 熱あるなら早めに病院行った方がいいよ。」

この文を初めて見たとき、そういう感じがして、それに基づいて訳してみました。

間違っていないならそれでオーケー。

@CK I think that translation could work for this sentence as well, if I'm understanding tommy_san correctly.

間違ってないです。「とっとと」はニュアンスのきつすぎる言葉ですが。
