What does this sentence mean? "Then" could be used to mean "at that time (in the past)" or "well, as the next topic".

I think if it referred to the past it would be "How old was he then (i.e. 'at that time')?"
I think this sentence in the present tense would be used in the context where someone guessed someone's age but got it wrong e.g. "Tom's 25." "No he's not." "(Well)how old is he then?".
