Can this mean "What's wrong with you?" or "What's wrong?" (What's your issue, what's bothering you)
The sentence kind of strongly implies that the problem is with the addressee, i.e. there's something wrong with them (maybe a screw in their head came loose). If you want to avoid coming across as rude, #903010 "Was ist das Problem?" would be a safer choice, or even #740975 "Womit kann ich dir helfen?"
