I'm not with you. Are you asking if the the Japanese is talking about a sister or a younger sister?

Japanese doesn't have a word that 'just' means sister. It's always either older sister or younger sister. So, usually, when it is translated into a language like English that isn't obsessed about 'younger' or 'older' all the time "younger sister" is just translated as "sister".
