Alternatively, it could be changed to:
His health is beginning to decline now.
Since this sentence is part of the Tanaka Corpus, I thought I'd pass it by you first.
In terms of matching the Japanese your first version is better. Slightly more literal would be "His health has already begun to decline".
The original is rather clunky.
