¿Qué modo? is use when you're asking for function, instruction, in short more on methodology.
for example: ¿Qué modo ta usa este móvil? how do you use this mobile phone?
¿Qué hechura? is use when you're asking the way how they look during a certain situation. e.g. how did he reacted from all criticisms
for example: ¿Qué hechura su cara cuando ya cae ya na escalera? How was his/her face when s/he fell down in the stair?
¿Qué laya? is the common "how"among the three since it's been using constantly. use when you're referring to the way how a person fell down or how to reach a certain place.
for example: ¿Qué laya ya reventa el cable? How did the cable break?
