I believe you should add two translations instead of one with annotations, namely:
Czy on kiedyś jadł indyka?
Czy ona kiedyś jadła indyka?

Don't include annotations as part of sentences.
Don't include things like the following inside your sentences :
He/she (as in He/she said it was hot.)
Instead, you should submit 2 sentences.
He said it was hot.
She said it was hot.

Instead, put this as a comment and ask for someone to add it as a tag (or if you have tagging rights, then tag it yourself).

A bit differently. This sentence should be changed to
Czy on kiedyś jadł indyka?
And you could add an alternative translation:
Czy ona kiedyś jadła indyka?
as a separate sentence

You can see there are two Polish translations (for he and she)
EDIT: funny how those two Polish translations I linked to as an example happened to be wrong, but at least they illustrate my point.
