The meaning is "one tends to speak about what's on their mind at the time (about what's worrying them)".
@neron: it's just a proverb on the topic of anxiety or neurosis. One tends to worry an aching tooth with one's tongue in the same way that the mind tends to frequently return to a personal problem or dilemma. That's my understanding, anyway.
I fixed the sentence, as tooths is not a word.
