This sentence seems ambiguous. Did Tom get someone to cut his head off, or did Tom go through the experience of having his head cut off?

It certainly is ambiguous when presented like this without enough context.
Henry Moore disobeyed the king. He (the king) had his head chopped off.

So actually this sentence has three meanings.
1. Tom ordered someone's head to be chopped off.
2. Tom got his head chopped off.
3. Tom ordered someone to chop his head off.
