- When used as an adverb, 'really' is usually translated as 真係 (more casual) or 真正 (more serious) but never 真.
- The 嗻 sounds a bit out of place here because it carries a tone of casual reassurance that is absent in the original English sentence.
- 媽媽 is correct but sounds a bit childish for this context (maybe it's just me, I don't know).

My suggestion:
真正明白我嘅人淨係得我阿媽一個。
