The 已 indicates that it was an ongoing problem stretching back in time, which is why I didn't use the past continuous. "Was struggling to breathe" would indicate that the situation had just arisen (as in, the horse was in an accident and was struggling to breathe there and then), not something that had been going on for a long time ("the horse is old and struggles to breathe.") The translation sounds acceptable to me, but I like the suggestion "had problems breathing," so I'll change it.
