This may just be me but I think "are you in love with my sister" would be more natural for someone asking about another's feelings toward their sister. "Do you love" is in my mind an inquiry about a less transient or deeper example of that feeling; something you might ask someone about their mother ... or brother-in-law about your sister (possibly adding a "still").

The usage examples were chosen as diametrically opposite as possible to illustrate the difference, not to imply a vast difference in meaning. If anyone has a better explanation of the difference, it would be interesting to hear it.
