What does this mean?

I only understood this English sentence after reading the German:)
1. You can feel the worm as a painful strand under the skin.

Here's definition #2 in the Concise Oxford English Dictionary:
strand2
■ noun
a single thin length of thread, wire, etc., especially as twisted together with others. ▶a single hair or thin lock of hair.
an element that forms part of a complex whole.
(...so a worm under the skin resembles thread or wire.)
