Do I need a comma after "know"?

A comma would be helpful to avoid ambiguity. Without a comma, the sentence can be interpreted as "You know Tom, so you're a real idiot". That's what I thought :)

Correct options: either two commas (before and after apellation) or no commas.
The sentence as it is has not too much sense.
(Or: You know Tom, he's a real idiot.)
